I have installed Oracle 9i and have been using it fine, but now when I try to start my Forms Bulider (Start > Programs > Oracle 9i Developer Suite - Oracle9iDS > Forms Developer > Forms Builder)
it gives me these errors:

    FRM-18103 Falied to Initialize the Development Environment
    FRM-10039: Unable to start up the Form Builder

I have already started the OC4J instance and don't know what the problem is. Any ideas?

I had the same problem. Finally figured it out:

On our PCs, the Oracle9i Client needed to be in
c:\oracle\ora92
and the Oracle Developer needed to be in
c:\oracle\oradev

Since the default put them both in the same ora92 directory, some of the files were probably overwritten.

If you accept the default directory names then this is not a problem. You must have the Database and Forms/Reports in separate 'homes' as well as directories. Make sure that the 'tnsnames.ora' files match. Search this forum for 'tnsnames.ora' for a fuller explanation.
